Athletic clearance for the 2026-27 school year is open! Athletic clearance is required each year, even for returning athletes. All athletic clearance paperwork must be submitted in order for a student-athlete to receive a Ticket to Play, (see below). Athletes will not be permitted to attend tryouts and/or practices until they have been cleared by the athletic department.

Select the 2026-27 school year, and Sage Creek High School. Students must complete the athletic clearance process by the due dates listed below.
Please do not drop off forms to the Athletics office.
1. New students: Click on the “Athletic Clearance Instructions” button and follow the instructions
Returning students: Click on the “HomeCampus.com” button and log into your account. Select “start clearance” to create a 2026-27 account. This is where you can select all the sports you/your student will be participating in. Most of the information will repopulate from last year’s clearance, so please verify and make any updates.
2. Once in Home Campus you will need to attach the required forms:
- CIF Physical Forms from link button above, (signed, stamped, and dated by a MD, DO, PA, NP licensed in the state of CA). Physicals are valid for 1 calendar year from the date of examination.
- Proof of Insurance uploaded to online registration. Athletes must be covered by health/medical insurance for the duration of the selected sports season.
Please note: If a student uses any medication (prescription or over the counter) during the school day and/or while participating in sports, the parent/student/prescribing health care provider must complete the CUSD Medication and Procedures forms. These forms need to be completed annually per California State Ed Code. Please attach them in the students Home Campus registration as part of the Physical Forms.
*Athletic Clearance registrations submitted after the due date may take an extended length of time to review due to responsibilities to current in season teams. Late submissions do not guarantee a tryout, as coaches may close their rosters at any point after tryouts begin. Please be patient as we work through the clearances.*
Once the student’s forms and home campus account have been reviewed, a digital Ticket 2 Play will be issued through the Minga App. A Ticket 2 Play is required to attend tryouts and/or participate, no exceptions. The Ticket 2 Play will appear as a “sticker” one week prior to tryouts. A roster of all cleared students will be provided to the Head Coach of the Program prior to tryouts. Athletes will not be permitted to attend tryouts and or practices until they have been cleared by the athletic department. Incoming 9th grade students who do not have access to the Minga app will use their email clearance confirmation from Home Campus as their Ticket 2 Play.
We’ve partnered with 828 Urgent Care to offer sports physicals during the last two weeks of May. This is a great option for families looking to complete the required physical before the clearance deadline. The cost is $35 per sports physical, and a portion of the proceeds will go directly to Sage Creek Athletics.

- You may call the number on the flyer the day you plan to attend to be added to a waitlist (allows you to wait from home).
